• minimum of 6 month waiting period for something major
• maximum of $1,000, $1,500, $2000 of work done per year ( depending on plan)
• larger Dentist network

Diamond Plan- $56.07 per person
Plan Maximum:  $2,000 Per Person Per Calendar Year/$1,500 Lifetime Ortho Max
Deductible: $150 lifetime deductible for Ortho
Office Co Pay: $25 Copay Per Person Per Visit

Platinum Plan – $52.01 per person
Plan Maximum:  $1,500 Per Person Per Calendar Year/$1,500 Lifetime Ortho Max
Deductible: $50 per person per calendar year. Separate $100 lifetime for Orthodontic Procedures
